In order to provide the level of service that our clients expect and deserve, Certified Patient Services, LLC needs to conduct a rigorous, in-depth assessment of all aspects of their health, lifestyle, and environment.  To make sure we have what we will need to accomplish that, we ask that prospective clients first download the New Client Pack here or from the link on the right and then complete the New Client Information form, which you will complete and bring with you to your first office visit.  It will not be necessary to bring any medical records with you at this time, but if you have particular records that you think might be useful to illustrate your case, you are welcome to bring them along, as well.

Your initial office visit will focus mainly on getting to know each other and making sure both you and we have the information we both need to proceed.  After your visit, our team will review and evaluate your case, conducting whatever preliminary research is necessary to be able to outline a logical path to get you from where you are to where you wish to be.  This is the initial assessment, and we will review this with you at your next office visit.

It is important to remember that you will be under no obligation whatsoever to hire us to manage your case at any point during this initial phase.  The rest of the services listed here, however, are provided only under contractual agreement between us and the client.
