When a patient is released from a care facility, their exposure to mistakes and problems generally increases.  That is because, first, the stack of discharge paperwork is often a daunting one, so much so that it often goes at least partly unread.  And second, there is often no one who manages this period of transition, whether it be a move from one facility to another or from a facility back to their or a caregiver’s home.  The family, or even the patient themself, is tasked with coordinating the transfer of care, and they are often ill-prepared to take on such a burden, especially at that time.

Certified Patient Services, LLC can thoroughly review the instructions from the discharging provider and make sure that the patient’s needs are met not only during the crucial transportation phase when they may be devoid of care altogether for some period of time, but also at the moment they arrive at their destination.  Lack of readiness on the part of the receiving facility or household can cause delays in reestablishing the proper level of care that the patient requires.  It can also cause stress and confusion for everyone involved while staff or family members scramble to get what is needed.  By conferring with those parties ahead of time, we can eliminate these problems by making sure everyone is on the same page and prepared well in advance of the patient’s arrival.
