It could just be poor communication on the part of your provider — they’ve heard your concerns, looked into them, but for whatever reason don’t discuss their findings with you. Transfer of care is a grossly overlooked area of the healthcare continuum. It is supremely important to understand all of the discharging provider’s post care instructions prior to signing off on the discharge paperwork. Not only that, the provider your loved one is transferring to (another facility or into your care) must be prepared to accept them in their condition at that time and must have whatever is needed to care for them on hand and at the ready.
Ideally, you or the receiving facility would get an advance copy of the discharge instructions so you/they can be properly prepared. This rarely happens, however. Certified Patient Services can step in to get these instructions prior to your loved one’s release, simplify the terminology so they know what they are signing, and ensure that you or the receiving facility are adequately prepared prior to discharge time.
